• Voluntary intake form responses (see Part II for health-related data).

3. Third-Party Services.  

We utilize Square, Inc. for online booking, client communications, and payment processing. Square may collect and store certain personal data on our behalf.  You can view their privacy policy at: https://squareup.com/legal/privacy.  We do not sell or share your personal information with other third parties or advertisers.

4. Data Security

We employ reasonable safeguards to protect your personal information. All data is stored securely through industry-standard platforms, including Square’s encrypted infrastructure.

• Request a copy of your personal data.

• Correct any inaccuracies.

• Request deletion of your information (subject to legal obligations).

Part II: Consumer Health Data Addendum (Washington Residents)

This addendum applies specifically to residents of Washington State under the My Health My Data Act (RCW 19.373).

We may collect the following Consumer Health Data (CHD) from you:

• Health history (injuries, chronic pain, allergies, tension).

• Treatment preferences (e.g., pressure level, product sensitivity).

• Wellness goals related to your massage.

• Emergency contact information.

2. Why We Collect This Data

We collect CHD only to:

• Evaluate treatment suitability and safety.

• Customize your massage session.

• Maintain accurate records in accordance with Washington law.

3. How We Obtain Your Data

We collect data directly from you when you:

• Complete online intake or consent forms.

• Provide information through our booking platform (Square, Inc.).

4. Who We Share It With

We do not sell, rent, or disclose your health data. Your CHD is shared only with:

• Square, Inc., our booking and recordkeeping provider.
